William Melvin Fleming sent a Petition For Initiation to Temple Lodge No. 33 on 11 August 1924. It stated he was born in London, Ontario on 18 November 1887, that he was then residing in Duncan, his mailing address was Box 235, and his occupation was District Agriculturalist.
He was sponsored for membership in Temple Lodge No. 33 by Lucien Brockway and Charles William ONeill.
The Committee of Investigation, consisting of William Mordaunt Dwyer and two others whose signatures are illegible, reported favourably and William Melvin Fleming was duly elected to membership in Temple Lodge No. 33 by Initiation.
