Thomas Lyell Forrest sent a Petition For Affiliation to Temple Lodge No. 33 on 11 November 1924. It stated he was born in Kirriemuir, Scotland, was residing in Duncan and was aged 51. He stated that he had previously been a member of Lodge St. Ninian No. 732 in Alyth, Scotland.
He was sponsored for membership in Temple Lodge No. 33 by George Henry Savage and Charles William ONeill.
The Committee of Investigation, consisting of Thomas Pitt, Lucien Brockway and Alfred Edward Green reported favorably and Thomas Lyell Forrest was duly elected to membership in Temple Lodge No. 33 by Affiliation on 10 February 1925.
He dimitted from Temple Lodge No. 33 in 1930.
